Health Links is a new model of care that makes it easier for people who rely on our system the most to access health care in Ontario. Health Links is centered on one simple concept: physicians, specialists, hospitals, home care, long-term care and community services working together with patients, families and caregivers to better coordinate care.
This is a collaboration of 20 partners across the continuum of care to form the Southwest York Region Health Link to help make it easier for people who rely on the system the most, such as seniors and people with complex conditions, to access health care in our region.
Southwest York Region Health Link patients receive intensive support to help them stay healthier in their own home. They are connected to a dedicated Health Link Care Coordinator, participate in a case conference with their care team and contribute to the development of a coordinated care plan based on their unique goals.
